Our Service Area:
VAN provides services to residents of the following counties/cities in the central Shenandoah Valley:
- Rockingham County (including the city of Harrisonburg)
- Augusta County (including the cities of Staunton & Waynesboro)
- Rockbridge County (including the cities of Lexington & Buena Vista)
- Bath County
- Highland County
- southern Shenandoah County
- western Page County
Basic Client Eligibility Qualifications:
- Client must have tested positive for HIV/AIDS.
- Client's income must be within 300% of the current Federal Poverty Level (FPL)
- Client must provide verifiable proof of income or financial support.
Client Services:
- Individualized HIV Case Management*
- Community Resource referrals
- Assistance with co-payments for medical services and HIV-related or other non-HIV formulary approved medications.
- Emergency Food assistance
- Arrange volunteer transportation to HIV medical appointments in Charlottesville **
- Emergency rent or utility payment assistance (gas for heating and electric only) ***
- Monthly Social support Groups (English & Spanish)
* HIV Case Management - assistance provided in partnership with a client living with HIV/AIDS to identify and facilitate access to and utilization of resources required to achieve and maintain the highest level of independence possible.
** Transportation - Clients on Medicare programs providing transportation are not eligible to seek additional transportation assistance through VAN. All arrangements for client rides to Charlottesville are based on volunteer availability and all other means should be used to find suitable transportation alternatives PRIOR to contacting VAN. If a client needs a ride, they must contact their Case Manager at least 10 days prior to the appointment. If VAN is unable to locate a volunteer it is the CLIENT'S responsibility to cancel and reschedule the appointment.
Case Managers do NOT provide client transporation.
*** Emergency Financial Aid (EFA) funds are NOT entitlement funds and are extremely limited or unavailable. VAN should always be FUNDING OF LAST RESORT after other private and community financial assistance resources have been contacted.
