SERVICES

Our Service Area:

VAN provides services to residents of the following counties/cities in the central Shenandoah Valley:

  • Rockingham County (including the city of Harrisonburg)
  • Augusta County (including the cities of Staunton & Waynesboro)
  • Rockbridge County (including the cities of Lexington & Buena Vista)
  • Bath County
  • Highland County
  • southern Shenandoah County
  • western Page County

Basic Client Eligibility Qualifications:

  • Client must have tested positive for HIV/AIDS.
  • Client's income must be within 300% of the current Federal Poverty Level (as of Feb 2008)
  • Client must provide verifiable proof of income or financial support and other supporting documentation as required before services can be provided. 


Client Services:

  • Individualized HIV Case Management*
  • Community Resource referrals
  • Copayment assistance for HIV-related medical services, HIV medications and/or approved non HIV medications.
  • Copayment assistance for routine dental checkups, cleanings and fillings).  We do not provide financial assistance for crowns, dentures, whitening or any other restorative procedures
  • Copayment assistance and referrals to Mental Health services
  • Nutritional Supplements (with physician prescription)
  • Emergency Food Bank (once a month)
  • PARTIAL emergency rent or utility payment assistance (gas for heating and electric only) *** 
  • Free condoms, lubrication, pill boxes
  • HIV health education and risk reduction counseling
  • Treatment Adherence support
  • Monthly psychosocial support Groups

* HIV Case Management - assistance provided in partnership with a client living with HIV/AIDS to identify and facilitate access to and utilization of resources required to achieve and maintain the highest level of independence possible.


** Transportation - Clients on Full Medicaid (which provides hospital   transportation through LOGISTICARE) are NOT eligible to seek additional transportation assistance through VAN.  If a client needs transportation assistance, they must contact their Case Manager at least 10 days prior to the appointment.  If VAN is unable to locate a volunteer it is the CLIENT'S responsibility to cancel and reschedule the appointment. 

 Case Managers DO NOT provide client transporation.

*** Emergency Financial Aid (EFA) funds for rent or utilities are extremely limited or unavailable. VAN may offer PARTIAL assistance and should always be FUNDING OF LAST RESORT after other private and community financial assistance resources have been contacted
